POSITION: Family Nurse Practitioner (Exempt)
REPORTS TO: MD
AREAS OF RESPONSIBILITY:
Under the direct or indirect supervision of the preceptor physician and under the medical protocols and standardized procedures on file at the facility where working, see clients presenting at the facility seeking medical services.
Diagnosis and treatment of clients presenting will be within the framework of the Family Nurse Practitioner’s skill and knowledge base, and within the standardized procedures and protocols developed for the facility.
Diagnosis and treatment that falls outside of skill level and or protocols shall be handled in the following manner:
1. Phone consultation with Preceptor physician, or,
2. Reschedule back to be seen by Preceptor Physician, or,
3. Referral to specialty Physician in referral network, or,
4. Referral to hospital emergency room
In addition the FNP is responsible for the supervision of medical staff and auxiliaries staff under them at the facility where they are on duty.
QUALIFICATIONS: Licenses as a registered nurse in the state of California. Certified by the California Board of Registered Nursing to hold out as a Family Nurse Practitioner in the state of California. Desired extra qualifications, not required for employment. Certification as a FNP by examination through the American Nurses Association. Ability to work in a fast paced environment, prioritize tasks, be flexible to meet the needs of patients and co-workers, and use discretion/good judgment. Must maintain confidentiality relating to MVHC personnel, patients, and information, which could result in proprietary damage to MVHC. Must possess current valid California Drivers license and auto liability insurance.

REQUIREMENTS AFTER HIRE:
Maintain credentialing and privileging requirements which include active and good standing of licensure.
Maintain CPR, Annual TB Skin Testing, Annual Required Trainings for FTCA Coverage
Maintain Auto Insurance and active Driver’s License
DUTIES AND RESPONSIBILITIES:
1. Patient Assessment and Physical Exam:
o Conduct routine checkups, screenings, and health assessments.
o Obtain medical history, including lifestyle habits, symptoms, and previous illnesses.
o Perform physical exams and evaluate the patient's overall health status.
2. Diagnosis and Treatment:
o Diagnose common acute and chronic conditions (e.g., hypertension, diabetes, infections).
o Develop treatment plans based on medical diagnoses.
o Prescribe medications or treatments, as needed, and adjust based on patient progress.
3. Preventive Care:
o Provide guidance on health promotion, lifestyle changes (e.g., diet, exercise), and stress management.
o Offer vaccinations, screenings (e.g., cancer, cholesterol), and preventive interventions based on age and risk factors.
o Educate patients about disease prevention and healthy behaviors.
4. Chronic Disease Management:
o Monitor and manage long-term conditions such as asthma, diabetes, heart disease, and arthritis.
o Adjust medications and treatment plans as necessary to maintain disease control.
5. Referrals and Coordination:
o Refer patients to specialists, diagnostic testing, or other healthcare providers when needed.
o Coordinate care with other healthcare professionals (e.g., physical therapists, dietitians) to provide comprehensive care.
6. Patient Education and Counseling:
o Counsel patients on managing health conditions and making informed decisions about their care.
o Address concerns and answer questions related to conditions, medications, and lifestyle changes.
7. Documentation and Record Keeping:
o Maintain detailed and accurate medical records of patient visits, diagnoses, treatments, and follow-up plans.
o Documentation must be completed within 72 hours of the patient encounter.
o Update records to reflect patient progress and changes in treatment plans.
8. Emergency Care:
o Provide first-line emergency care for acute or urgent medical issues when necessary (e.g., injuries, infections, allergic reactions).
o Assess whether the patient needs to be transferred to the emergency department.
9. Collaboration with Medical Team:
o Work alongside nurses, medical assistants, and other healthcare professionals to ensure holistic care.
o Attend team meetings and contribute to quality assessment/improvement initiatives to improve the health of patients and community.
10. Follow-up Care:
o Schedule follow-up appointments to monitor patient progress, discuss lab results, or address concerns.
o Ensure continuity of care by ensuring that treatment plans are carried out and modified as necessary.
11. Administrative and Compliance Duties:
o Ensure adherence to health regulations, including HIPAA, and maintain ethical standards in patient care.
o Stay updated with new medical research, treatment guidelines, and best practices.
